Why do I need to provide my credit card information?

Even if your insurance is in-network with Monument, there are certain scenarios in which your plan might not fully cover your care, requiring additional payment.

It’s our goal to provide as transparent pricing as possible, which is why we ask you to fill out the Insurance Information Form, and estimate plan costs and coverage on your behalf. However, there are situations where insurance providers will not cover care, such as if a deductible is not yet met, or if there is a redetermination of your benefits. We will do our best to notify you in these scenarios, and will alert you before charging your credit card.
